Do you need the PTO to purchase something for you?

Teachers:  Use this form if you need to request a purchase outside of the Amazon website.  If your item can be found on Amazon, please add it to your PTO Amazon Wishlist.  If you need help accessing your wishlist, please email info@brackettpto.org.

Parent/Caregiver Volunteers:  If you are purchasing an item for a school event or classroom activity, you may use this request form and the PTO will buy the item(s) and have them shipped directly to the school.  You may pick up your item in the front office once it is delivered.  Committee/Event Leads: If your item can be purchased on Amazon, please add it to your Committee/Event’s  Amazon Wishlist.  If you need help accessing your wishlist, please email info@brackettpto.org.
